The date when a Member's Personal Benefits become effective is set forth in the paragraph with the title Effective Dates of Personal Benefits.
The date when a Member's Dependent Benefits become effective is set forth in the paragraph with the title Effective Dates of Dependent Benefits.
The amounts of coverage are determined by the paragraph with the title Schedule of Benefits.
Limitations apply for residents of the following states: Arkansas, California, Florida, Georgia, Maryland, Texas, Utah, Virginia, West Virginia and Wisconsin.
